ACHP Welcomes Dr. Connie Hwang as Chief Medical Officer and Director of Clinical Innovation
Washington, D.C. (January 30, 2018) – The Alliance of Community Health Plans (ACHP) has named Dr. Connie Hwang, MD, MPH, Chief Medical Officer and Director of Clinical Innovation. As the leader of ACHP’s Clinical Innovation team, Hwang will direct efforts on higher quality standards and advise ACHP’s patient-centered advocacy agenda.
An experienced physician leader, Hwang brings a strong clinical and population health perspective that will enhance ACHP’s mission of providing high-quality, affordable coverage and care. Working with ACHP clinical and quality leaders, Hwang will help drive health plan performance improvement as well as spearhead ACHP research and programming on patient experience, delivery transformation and community benefit programs. Hwang will also inform efforts with Washington policy and decision makers as ACHP demonstrates the impact of nonprofit, community health plans.

Hwang comes to ACHP from Evolent Health, a population health management company focused on the transformation to value-based care, where she served as Vice President, Quality and Clinical Performance. Hwang has also held senior positions at the National Quality Forum and Resolution Health, Inc., and served on two National Quality Forum expert committees. She is a licensed internal medicine physician who completed her research fellowship and Master in Public Health at Johns Hopkins Hospital and Bloomberg School of Public Health.
