ACHP plans recognize the importance of taking a community-wide approach to improving health. That’s why they work with local partners to engage and educate their communities on strategies for healthier living.
AvMed – Healthy Aging
AvMed deploys a variety of programs aimed at helping seniors stay healthy in mind, body and spirit: Club ASPIRE provides an environment that fosters member connections; AvMed CONNECT helps community members connect with common interests; and AvMed Access is a comprehensive care management program where trained medical professionals offer support for chronic and acute conditions.
Martin’s Point – Healthy Aging
Headquartered in Maine, the nation’s “oldest state” (population-wise), Martin’s Point Health Care is paying special attention to the wellbeing of an ever-growing senior population. Martin’s Point has partnered with the Southern Maine Agency on Aging to host the annual Maine Senior Games and is helping seniors connect with peers and stay active through community centers at primary care facilities.
Security Health Plan – Healthy Aging
Security Health Plan has launched multiple programs to support healthy aging. Security Health Plan partnered with the Wisconsin Institute for Healthy Aging (WIHA) in 2016 to offer Stepping On, a seven-week program that reduces falls by 50 percent. In 2016, Security Health Plan also gave local organizations nearly $100,000 in Healthy Aging Grants to help older adults overcome chronic disease, falls, lack of exercise and difficulty understanding drug labels.
Kaiser Permanente – Thriving Schools
Thriving Schools is a national effort designed to influence long-term behavior change and have a measurable effect on the health of students, staff and teachers in K-12 schools in communities served by Kaiser Permanente. It is focused on supporting healthy eating, active living, school employee wellness and positive school climate through policy, systems and environmental changes.
HealthPartners – PowerUp
HealthPartners’ PowerUp is a community-wide youth health initiative to make better eating and active living easy, fun and popular. PowerUp works at multiple levels in a collaborative and comprehensive approach to create community change.
Independent Health – Fitness for Kids
The Independent Health Foundation sponsors the Fitness for Kids Challenge to help combat childhood obesity in the Buffalo, NY community. As part of the seven-month initiative, teachers are given in-class and at-home challenges and participating schools are asked to make a health policy changes in their buildings, such as replacing unhealthy snacks in school vending machines.
UCare – Minne-Loppet Youth Skiing Program
Recognizing the importance of developing healthy habits in youth, as well as addressing childhood obesity, UCare supports the Minne-Loppet program, which introduces cross-country skiing and healthy eating to inner-city youth at Minneapolis elementary schools as an adjunct to the schools’ physical education classes.
