Regional, Nonprofit Health Plans Lead in 2025 NCQA Quality Ratings

ACHP member plans receive top ratings across all lines of business

Washington, D.C. (November 4, 2025) – The 2025 NCQA Health Plan Ratings confirm, once again, that the Alliance of Community Health Plans (ACHP) member companies set the standard for high-quality, community-based care nationwide. NCQA’s ratings show that across commercial, Medicare and Medicaid contracts, ACHP members outperform non-ACHP plans on the vast majority of measures, with particularly strong results in care coordination, chronic disease management and maternal and reproductive health.

Both Medicare plans awarded NCQA’s prestigious 5-star rating in 2025 are ACHP members, including Kaiser Foundation Health Plan and Network Health. Similarly, six of the seven top-rated commercial contracts to receive 5 stars are ACHP members. These contracts include Kaiser Permanente, Independent Health and UPMC Health Plan.

These achievements exemplify the consistent excellence and innovation that sets ACHP members apart from other health plans, highlighting their leadership in providing high-quality, patient-centered coverage and care.

“ACHP members don’t settle for merely meeting benchmarks—they exceed them and make meaningful impacts on the health of the communities they serve,” said ACHP President and CEO Ceci Connolly. “From enhanced chronic disease management to coordinated care, ACHP members continue proving they deliver what matters most: good health.”

Commercial

ACHP commercial plans continue leading the industry, scoring nearly a full point higher than the industry average. ACHP plans delivered strong clinical quality, scoring above 4.0 out of 5.0 on key measures including prenatal checkups, diabetes eye exams and blood pressure control for patients with diabetes.

This commitment to preventive care is underscored in ACHP’s Chronic Disease Pledge. The pledge marks a bold long-term commitment from ACHP members to address and reduce the incidence of diabetes and hypertension in their communities. By addressing social determinants of health and implementing innovative programs, ACHP members are already witnessing significant improvements in population health.

Thirty-eight ACHP commercial contracts achieved a rating of 4 stars or higher, representing offerings from Aspirus Health, CDPHP, Geisinger Health Plan, Group Health Cooperative of Eau Claire, Group Health Cooperative of South Central Wisconsin, Health Alliance, Health New England, HealthPartners, Independent Health, Kaiser Permanente, Martin’s Point Health Care, Mass General Brigham Health Plan, Network Health, Priority Health, Providence Health Plan, Sanford Health Plan, Security Health Plan, Select Health and UPMC Health Plan. Kaiser Permanente, Independent Health and UPMC Health Plan earned the highest possible rating of 5 stars.

Medicare

ACHP Medicare plans maintained exceptional performance, beating non-ACHP plans on nearly every measure and achieving an overall rating nearly half a star above the industry average. These plans excelled in care coordination, appropriate antibiotic use and mental health follow-up. Several measures exceeded non-ACHP performance by more than one full rating, including measures related to appropriate antibiotic use and transitions of care.

Twenty-five ACHP Medicare contracts earned 4 stars or higher, including plans from AultCare, CDPHP, Geisinger Health Plan, Health Alliance, Health Alliance Plan, Kaiser Permanente, Network Health, Priority Health, Providence Health Plan, Sanford Health Plan, Security Health Plan, UCare and UPMC Health Plan. Kaiser Permanente and Network Health each received the top rating of 5 stars.

Medicaid

ACHP Medicaid plans continued displaying strong quality outcomes across most measures, outperforming non-ACHP peers with overall ratings more than half a point higher. Plans showed particularly notable results in chronic disease management and preventive care, exceeding non-members by more than a full rating on measures such as diabetes kidney health evaluation, blood pressure control and Td/Tdap immunizations for adults.

“Our member plans are consistent, hitting the highest quality marks, no matter the line of business,” said Connolly.

Sixteen ACHP Medicaid contracts earned 4 stars or higher, representing plans from CDPHP, Fallon Health, Geisinger Health Plan, Health New England, Kaiser Permanente, Mass General Brigham Health Plan, Network Health, Priority Health, Security Health Plan, Select Health, UCare and UPMC Health Plan.
