Improve Diabetes Outcomes

Today more than 30 million people in the U.S. have diabetes, though as many as 25 percent of them don’t even know they have it. Diabetes puts patients at higher risk for heart disease and stroke and is the leading cause of kidney failure, lower limb amputations and adult-onset blindness. In order to address this mounting public health crisis, Minnesota-based HealthPartners has developed new processes for identifying, treating and managing diabetes, leading to better care outcomes.
