Chronic Disease Pledge

The Chronic Disease Pledge is a 10-year commitment by ACHP’s Board of Directors to make progress toward reducing the incidence of chronic disease.

ACHP members signed the Chronic Disease Pledge, committing to address and reduce the incidence of diabetes and hypertension across the country. Our member plans have outlined specific and tailored goals for their communities to address chronic disease through a social determinants of health lens, implementing programs and expanding educational initiatives.

The origins of this pledge are in Health Care 2030: ACHP’s Roadmap to Reform, a forward-looking report outlining practical changes for immediate and long-term improvement in the U.S. health care system.  

Since the Chronic Disease Pledge was signed in 2023, ACHP members have already witnessed significant improvements in the well-being of their populations.
