Health inequities result from a complex combination of clinical, economic, environmental and social factors. Advancing health equity requires health care organizations to intervene across multiple, interwoven levels of intervention, including the individual, community and systemic level. This framework demonstrates how a holistic approach with interventions action across all the action loops is necessary to truly improve equity in health care.
